Instant Neck Lift

As  minimally invasive facial rejuvenation technologies improve we start getting closer to obtaining surgical like results without a scalpel.  At the present there is not a single non/surgical or minimally invasive  modality that seems to get the results people were looking for in a nonsurgical neck lift.  I have combined, micro-Lipo suction, internal injectable radiofrequency (ThermiTight), and barbed absorbable suspension sutures to create and almost instant result that improves with time and usually last several years. The procedure that I call "The Instant Neck Lift"  is performed under local anesthesia and usually takes less than an hour to complete.<br />

The patient goes home with a compressive dressing which is removed.  The next morning and then they wear it at bedtime only for approximately 2-3 weeks.  Bruising is minimal because we use only blunt needles and normal activities can be resumed almost immediately with the exception of heavy exercise that may affect the effectiveness of the barbed sutures.<br />

The more we learn about our body's ability to stimulate healing and regenerate itself,  the more we find innovative ways to restore the failing parts of her body.  <a data-cke-saved-href="http://www.dr-pena.com/prp-platelet-rich-plasma-and-fat-replacement-options.html" href="http://www.dr-pena.com/prp-platelet-rich-plasma-and-fat-replacement-options.html" style="color: #0782c1;"><u><em><strong>Platelet rich plasma</strong></em></u></a> also known as PRP is a highly concentrated suspension of our own bodies healing triggering bodies known as platelets.  They contain a myriad of hormones or 'factors' that begin and stimulate the process of 'wound healing' and repair in many different ways.  For hair stimulation the <u><strong>epithelial growth factors </strong></u>are the most valuable.  The stimulate the hair follicles in 2 ways.  First is  to increase the circulation to the follicles  at critical moments in the development of the hair thus making the hair thicker (up to 40%)  and 'waking' up some of the hair follicles that were in the resting or dormant phase (>20%). </div>

The Platelet Rich Plasma (PRP) is made by drawing your blood and centrifuging or spinning this blood to concentrate the platelets and remove the red blood cells.  This allows only the stimulating platelets without the remaining parts of the blood which can have an irritating the effect.</div>

A recent study performed using 3-D imaging concluded that it takes an increase of 130 ccs to 150 ccs to increased a woman's breast by 1 size.  This is approximately 4-1/2 ounces of volume increase.  I have used this formula for many years before this was confirmed by this study based on my interpretation and personal history of having performed 2000 or so <i><u><a href="http://www.dr-pena.com/breast-augmentation.html">breast augmentations.</a></u></i> <br />

The most common comment to the here after breast augmentation is "I wish I had gone bigger".  The psychology of this is based on the fact that the body image changes over a 3-6 month period.  At that point what ever size you are  you feel like you have always been that size.  I often spent 30 minutes or more going through the 3-D visualization of a prospective patient with their new implant options.  After reviewing 5 or 10 different sizes and how they would look on that individual, they asked me "Dr. what size is that?".  when I tried to explain that I have no idea but that if it looks good then that is the one that they should go with they tell me they want to be a specific size.</div>

Commonly, I have been asked to increase the current <b><a href="http://www.dr-pena.com/blog/infographic-all-about-breast-implants.html">implant </a></b>by 1 size or 2 sizes and I used the above formula.  That works a little bit more efficiently because the natural breast compression that occurs during the initial breast augmentation has already occurred.  What I mean is that when you have led stay a 300 cc natural size breast and you put a 300 cc implant you do not get a 600 cc size breast.  What happens?  The breast tissue is quite compressible whereas silicone and saline aren't.  The final result may be anywhere from 450 cc to 550 cc depending on the compressabilty of the natural breast itself.</div>

Even 10 years ago there was only surgical solutions which included liposuction (of one form or another: tumescent, ultrasound assisted , or laser assisted) or actually performing a neck lift where the fat is actually removed surgically and the skin is tightened!  Even today these two surgical methods offer the most efficient and consistent way of dealing with a problem.  The trend  today, however,  is to avoid surgery and embrace nonsurgical options. Here are the most noteworthy:<br />

These options include: </h3>

Radiofrequency </h4>
Fat ablation and skin tightening using radiofrequency.  <b><i><a href="http://www.dr-pena.com/thermitight.html">TherniTight)</a></i></b>, while not "noninvasive" since it involves administering local anesthesia and using a needle underneath the skin, it is definitely and nonsurgical procedure.  Radiofrequency energy is administered  through the needle and heats up the surrounding tissues and overlying skin.  If the temperatures reach 70°C the fat tissue is destroyed and the overlying skin is tightened.  Lower temperatures are used to just tighten the skin. This is a gradual process that may take up to 3-6 months to see the final results. The sister procedure ThermiSmooth can be totally noninvasive and the radiofrequency energy is applied through the skin.  Many other systems  such as  Exilis/ Thermi250, etc use the external  non invasive approach .   These however are much less efficient at fixing the double chin dilemma since the temperature that is required to affect the fat and even the skin can be quite high and somewhat uncomfortable.</div>

Microfocused Ultra sound </h4>
 Microfocused ultrasound is one of the later entries into the field. The device approved in the US is "Ulthera".  While it has enjoyed some success the discomfort during treatments  and unpredictable results have limited it from being totally embraced by the aesthetic community.  This uses focused medium frequency ultra sound energy to "heat up" fat cells by causing them to vibrate. When the "magic" temperatures between 65 and  70°C is reached the fat is destroyed and the body eventually starts an inflammatory process to get rid of it.  It too requires weeks to months to get final results.</div>

 Freezing the Fat </h4>
has now been adapted to treat the double chin or submental area.  Zeltiq/ Coolsculpting as it is known has been one of the fastest growing masses of nonsurgical fat removal over the last 5 years.  Generally if you can fit whatever area you want to minimize into one of the heads of the machine then he will have some a fact.  By freezing the fat it has a same effect as if it were reaching the magic 70°C but in the other direction.  The fat also died is 1 it is frozen.  The body will take it away over the next few weeks to few months.  The question is how much skin tightening in the overlying tissue will occur and how efficient will it be!<br />

The post procedure discomfort is <strong>MINIMAL</strong> and downtime is also. Lifestyle lift abruptly closed its doors after the model business platform failed a midst unhappy patients and reprimands for misrepresenting the truth in advertising. While there was no franchise in Naples, I saw several 'less than happy" patients that had undergone Lifestyle lifts in the Ft Myers office. While you can say every doctor has some unhappy patients, these chose not to go back at all even if it the 'fix' was going to cost more than the original service. While I try to treat everyone fairly, I do charge for my services. I will try to minimize any of those patients that were unhappy or "abandoned' by the closure of the Lifestyle Lift "Chain".<br />

It would seem  to be counterintuitive for a plastic surgeon to write an article on how to avoid a face lift.  Frankly we probably know  more about the subject than anybody else since we've watched our patients literally  grow up and grow older in front of our eyes.   Not only are we trained observers, but we also have the knowledge and tools to slow down (and in some cases reverse) the aging process. A <a href="http://www.nbcnews.com/id/33385839/ns/health-skin_and_beauty/t/twin-study-reveals-secrets-looking-younger/#.VMvUENLF_To" rel="nofollow">recent study</a> performed by a plastic surgeon who made a point to visit an identical twin convention in Twinville, Ohio,  revealed a great deal of information concerning non-genetic factors contributing to the aging process.  First of all you can not deny the influence of genetics on the aging process.  We all have  known  individuals that appear to be much younger than others the same age that take care of themselves in approximately the same manner. There are factors, however,  that will make <b><i>even identical twins </i></b>look significantly different by the time they reach their 40s 50s and 60s. If you are young enough to learn from these twins mistakes then read on.<br />

Environment Overriding Genetics!</h3>

1)<i><b> sun exposure</b></i>: there is no doubt that sun worshiping or unprotected exposure during outdoor activities will significantly affect how old you appear.<br />

4) <b><i>stress or the avoidance of stress</i></b> has a significant role in how you look. The divorced women that participated appeared to be 1.7 years older than they are married  identical twins! Paradoxically, widows looked two years the younger than they are still married twins.  While marriage can be stressful apparently divorce and financial difficulties can impact negatively on our aging process.<br />

5) <b><i>smoking</i></b> has long been known to affect the aging appearance.  The classic lines around the lips known as "smokers lines" as well as increase in generalized wrinkling and loss of elasticity are related to the direct negative effect on collagen elastin of tar and nicotine.<br />
<br />
6)  <b><i>excessive alcohol consumption</i></b> speeds up aging despite the current believes that her an ingredient in Red wine known as Resveretrol.  While a glass of wine may be good for your heart, too much can affect the liver and this will have a detrimental effect on the blood vessels in your skin as well as direct damage to your collagen.<br />

While these results give us information on what not to do in the future they do not shed light on what is available to 'turn things around' now that you're here.
